✔️ Indications

Olic 10 reduces the production and build-up of bile in the liver and also reduces the inflammation of the liver.

✔️ Pharmacology

Obeticholic acid is an agonist for the Farnesoid X Receptor (FXR), a nuclear receptor expressed in the liver and intestine. FXR plays a central role in regulating bile acid synthesis, inflammatory responses, fibrotic processes, and metabolic pathways. Upon FXR activation, the intracellular concentration of bile acids within hepatocytes decreases through:

• Suppression of de novo bile acid synthesis from cholesterol.
• Enhancement of bile acid transport out of hepatocytes.

These actions reduce the size of the circulating bile acid pool, promote bile flow (choleresis), and decrease hepatic bile acid exposure.

Pharmacodynamics

• FGF-19 Levels: Administration of Obeticholic acid (10 mg once daily) led to a 173% increase in concentrations of fibroblast growth factor 19 (FGF-19), an FXR-inducible enterokine involved in bile acid regulation, from baseline to Month 12.
• Bile Acid Levels:
  • Cholic acid concentrations decreased by 2.7 micromolar from baseline.
  • Chenodeoxycholic acid concentrations decreased by 1.4 micromolar from baseline.
    The clinical significance of these changes remains unclear.

✔️ Dosage of Olic 10

Adult Dosage for Primary Biliary Cholangitis (PBC):

Olic 10  is indicated for adults with primary biliary cholangitis (PBC) in the following cases:

• In combination with ursodeoxycholic acid (UDCA): For patients who have had an inadequate response to UDCA for at least one year.
• As monotherapy: For patients unable to tolerate UDCA.

Before starting therapy:

• Evaluate the patient's liver status using the Child-Pugh classification system (A, B, or C) to determine the appropriate starting dose.

1. Noncirrhotic or Compensated Cirrhosis (Child-Pugh Class A):

• Starting dose: 5 mg taken orally once daily.
• Dosage titration:
  • If an adequate response is not achieved after 3 months and the medication is well-tolerated, the dose may be increased to 10 mg once daily.
  • Maximum dose: 10 mg/day.

2. Child-Pugh Class B or C, or Patients with a History of Decompensation:

• Starting dose: 5 mg taken orally once weekly.
• Dosage titration:
  • If an adequate response is not achieved after 3 months and the medication is well-tolerated, the dose may be increased to 5 mg twice weekly (with at least 3 days between doses).
  • If further response is needed and tolerated, the dose may be titrated to a maximum of 10 mg twice weekly (at least 3 days apart).

• The maximum dose must not exceed 10 mg per day.
• Regular monitoring of the patient’s liver function and clinical status is essential during treatment.

Swallow Olic 10 Tablet as a whole with a glass of water; do not chew or crush it.

✔️ Avoid taking Obetohep-5 Tablet in the following cases:

• If you are allergic to any of its components.
• If you have a complete blockage of the biliary tract (affecting the liver, bile ducts, or gallbladder).

• Yellowing of the skin or eyes (jaundice).
• Black or tarry stools, or dark-colored urine.
• Coughing up or vomiting blood.
• Symptoms such as confusion, slurred speech, mood changes, anxiety, or swelling around the stomach area.
